Job Duties: - Provide medical care and coordination across various patient demographics. - Conduct comprehensive evaluations and diagnostic tests. - Develop individualized medical plans of care. - Schedule telehealth or in-person visits. - Participate in community clinics. - Prescribe medications and perform procedures. JOB QUALIFICATIONS: - Master's degree in family health. - Proficiency in Word, Excel, and EMR systems. - National certification as a Family Nurse Practitioner. - State-issued FNP license and Basic Life Support certification. - Preferably 3-5 years' experience in home or community health.
