**Responsibilities:** - Educate members on healthcare benefits via home visits and outreach. - Help members access care-related activities. - Document information and activities in relevant systems. - Provide and instruct on the use of Connections Plus® cell phones. - Coordinate and execute community events like Start Smart Baby showers and Birthday Parties. - Schedule and conduct home visits for various purposes such as noncompliance, benefits information, and ED diversion. - Make targeted phone calls for compliance, HEDIS initiatives, and health risk assessments. - Follow up on high-risk member referrals. - Connect members with community social services and agencies. - Travel within designated region as needed. **Education/Experience:** - High school diploma or equivalent. - Bilingual proficiency preferred. - 2+ years of experience in social services, community outreach, or education. **License/Certification:** - Valid driver's license and automobile insurance. - Some locations may require Community Health Worker certification, CPR certification, and/or bilingual proficiency.
