We are committed todelivering high-quality care that promotes independence and communityintegration for our clients.Position Overview:We are seeking a dedicated MSW (Master of Social Work) to join our team andprovide Comprehensive In-Home Care (CIC) and/or Independent Living SkillsTraining (ILST) services. The MSW will work with individuals directly to developand implement person-centered plans that support the individual's goals andpreferences, and promote independence and community integration.Responsibilities:▪ Conduct initial and ongoing assessments to identify the individual's needs, preferences, strengths, and challenges.▪ Develop and implement person-centered plans that address the individual's goals and preferences, and promote independence and communityintegration.▪ Provide ongoing support and coaching to individuals in their homes or community settings, as specified in the person-centered plan.▪ Assist individuals with daily living tasks such as meal preparation, medication management, and personal hygiene.▪ Provide training to individuals and their families on topics such as budgeting, household management, and communication.▪ Coordinate with other service providers involved in the individual's care, such as healthcare professionals, therapists, and case managers.▪ Implement and monitor Positive Behavioral Interventions and Supports(PBIS) strategies, in collaboration with the individual and their team.▪ Provide Independent Living Skills Training (ILST) to individuals to promotetheir independence and self-sufficiency.▪ Maintain accurate and timely documentation of services provided, progresstowards goals, and other relevant information.▪ Attend team meetings and other required trainings and meetings.Maintain confidentiality and adhere to ethical and professionalstandards.Qualifications:• Bachelor's degree in social work, psychology, or a related field.Experience working with individuals with traumatic brain injuriesand/or disabilities.• Knowledge of the NHTD program and regulations.Strong communication and interpersonal skills.• Ability to work independently and as part of a team.• Proficiency in computer skills, including electronic health record systems.
