We are seeking a dedicated Registered Nurse (RN) for our Home Health team in Houston, TX. You will be primarily field-based, providing care directly in patients’ homes and fostering strong relationships within the community.Key ResponsibilitiesConduct comprehensive clinical assessments of patients to evaluate their health status and needs.Develop, implement, and update individualized care plans in collaboration with patients, families, and healthcare teams.Administer medications and perform various medical procedures, including IV therapy and wound care.Document patient progress and adjust care plans according to changes in health status.Coordinate care and communicate effectively with multidisciplinary teams to ensure continuity of care.Provide education and resources to patients and their families regarding health management and treatment options.Ensure compliance with regulatory standards and best practices in all aspects of care delivery.Required and Preferred QualificationsBachelor of Science in Nursing (BSN) is required.State licensure as a Registered Nurse (RN).Clinical skills in areas such as Patient Care, Wound Care, and IV Therapy.Experience in Home Health or similar fields is preferred.Proficient in documentation and care coordination.Strong interpersonal and communication skills.At least 1-2 years of nursing experience in a clinical setting.Our Home Health team is structured to foster collaboration among nursing staff, therapists, and aides, all reporting to a dedicated Nurse Manager.
