Our work environment promotes collaboration, continuous learning, and professional development, making a significant impact on the health and wellness of our community.Key ResponsibilitiesAdminister medications and provide direct patient care, monitoring patient conditions and reporting any changes to the healthcare team.Assist in creating and updating patient care plans, ensuring all health needs are addressed effectively.Document patient information accurately in electronic health records (EHR) systems, maintaining confidentiality and compliance.Communicate effectively with patients and their families, providing education on care procedures and health management.Collaborate with a multidisciplinary team of healthcare professionals to deliver comprehensive care and support for patients.Perform basic health assessments by measuring vital signs and administering tests as directed by supervising RNs or physicians.Participate in staff meetings and training sessions to continuously improve patient care practices.Required and Preferred QualificationsActive LPN License in New Mexico.Strong clinical skills and attention to detail.Excellent communication and teamwork abilities.Proficient in patient care protocols and time management.Preferred: Experience working in a clinical setting for at least 1 year.
