These roles involve providing in-home therapy services to patients with the goal of restoring function, promoting independence, and enhancing quality of life.As a valued member of our interdisciplinary care team, you will conduct assessments, implement individualized treatment plans, and collaborate with caregivers and physicians to deliver exceptional, patient-centered care in the comfort of the patient’s home.Key Responsibilities:Conduct comprehensive evaluations and assessments in the home settingDevelop and implement customized care plans in alignment with physician ordersDeliver therapeutic treatments to support recovery, function, and independenceMonitor and document patient progress, reactions, and changes in conditionEducate patients and families on therapy goals, exercises, safety, and equipment useCollaborate with nurses, caregivers, and other professionals during case conferencesSubmit accurate, timely documentation per agency and regulatory standardsStay current with best practices through continuing education and participation in agency meetingsQualifications:Licensed as a PT or OT in the state of FloridaGraduate of an accredited Physical Therapy or Occupational Therapy programNBCOT certification required for internationally educated OTsStrong understanding of orthopedic, neurological, and post-surgical rehabilitationFamiliarity with adaptive equipment, mobility aids, and home safety practicesPrevious home health or outpatient experience preferred but not requiredReliable transportation to travel to patients’ homesWork Environment:In-home care setting (travel to various homes required)Conditions may range from well-organized to more physically demanding environmentsMust be able to lift, stand, bend, and operate standard therapy equipmentWhy Work With Us?At Pennie’s R Us Home Health Care, we value our therapists as essential partners in delivering holistic, personalized care.
