We are seeking a compassionate and reliable Certified Nurse Aide or Home Health Aide to provide essential personal care and supportive services to clients in their homes. This role is vital in helping patients maintain their dignity, privacy, and independence while delivering high-quality care outlined in the individualized care plan under the supervision of a licensed nurse.Please Note:Applicants must have at least one year of hands-on experience in a similar role. An open or flexible schedule is strongly preferred to meet the needs of our diverse patient population.Qualifications:High School diploma or equivalent preferredAbility to read, write, and follow directionsCurrent CNA certification in good standing (if applicable)Proven competency in performing caregiving tasksCompassionate, responsible, and dependable attitudeAt least 1 year of experience in home health, facility-based, or private care settingsFlexible availability is highly preferredEssential Responsibilities:Assist with bathing, grooming, dressing, and toiletingSupport mobility including transfers, ambulation, and prescribed exercisesMonitor and report vital signs and intake/output per care planPerform light housekeeping related to patient safety and comfortObserve, report, and document changes in condition or environmentFollow the care plan and RN instructions preciselyMaintain timely and accurate clinical notesReport any patient concerns to the supervising nurseParticipate in required staff meetings, orientation, and annual in-service training (minimum 12 hours)Adhere to HIPAA guidelines and uphold patient confidentialityWork Environment & Physical Demands:Travel to various patient homes with unique environmentsAbility to lift, bend, and assist patients with mobility as neededClear communication skills (verbal and written)Exposure to moderate noise and home equipmentWhy Work for Us?Join a dedicated and supportive team where your care has a meaningful impact every day.
