We are seeking a dedicated and compassionate Geriatric Home Care Nurse to join our team in Heredia, Costa Rica. This role is crucial for ensuring that our elderly clients receive the attention and medical support they need, resulting in meaningful improvements in their quality of life.Key ResponsibilitiesConduct comprehensive health assessments of geriatric patients, identifying their medical and emotional needs.Administer medications and treatments as prescribed, ensuring adherence to safety and health regulations.Develop customized care plans in collaboration with multidisciplinary teams to cater to the unique needs of each patient.Monitor and document patient progress, addressing any concerns with healthcare providers promptly.Provide emotional support and education to patients and their families to empower them in managing their health.Coordinate with physical therapists, social workers, and other specialists to ensure holistic care.Engage in regular training and development sessions to stay updated on best practices in geriatric care.Required and preferred qualificationsRegistered Nurse with a valid nursing license; additional certifications in geriatric care are highly preferred.Proven experience in nursing, particularly with elderly patients, in home care or similar settings (minimum of 2 years preferred).Strong knowledge of medical equipment and technologies used in home care.Excellent interpersonal and communication skills, both verbal and written, in English and Spanish.Ability to work independently and as part of a healthcare team, demonstrating strong decision-making skills.Familiarity with electronic health record (EHR) systems is a plus.In our organization, you will find a supportive team structure that values collaboration and the input of each member.
